Chase County Community Hospital & Clinics in cooperation with the Chase County Hospital Foundation offer a new health and wellness program titled WELLS Angels. The over all goal of this program is to improve general health and energy levels of participants, assist them in reaching the highest level of daily functioning possible and reduce potential cardiovascular events, stroke amputation and other consequences related to diabetes and cardiovascular disease.

Major Program Components

Wellness & Energy Plan

1)  Each particpant will have a complete health record search to reveiw prior care and determin current and future needs.

2)  Participants will be assessed to see if further testing is needed.

3)  Routine visits will be scheduled to check vital signs, receive foot exams and be evaluated for diabetic foot care.

4)  Participants will be scheduled for follow up visits based on need. Example: one month, three month, six month or annual.

Life, Laughter and Support Plan

1)  WELLS Angels Support Group:

  • Online Patient Portal:  Interested patients will be intorduced to eClinical Works web-based patient portal available to all  patients. Patients can log into system to view perscription information, lab results, diagnostice results as well as send and receive information from providers.

  • Social & Educational Activities:  As part of the WELLS Angels program, social and educational activities to be implemented include but are not limited to:

 - At least one social activity to introduce new participants to the program and celebrate achievements of those already particpating.

- Social Networking:  An electronic social media site (such as Facebook) will be established for those participangs who wish to stay in contact and support each other in their health efforts.

- Educational: Educational training will help participants learn how to use the Patent Portal, work with social networking and most importantly focus on health topics related to diabetes and cardiovascular diseases.

- Nesletter:  A quarterly newsletter provided to all participants will contain health tips and upcoming events related to the WELLS Angels program.

Costs Associated With WELLS Angels

Costs for participation will very based on each particpant's needs. Many insurance companies will cover costs associated with required testing, medication and rpeventative care related to the WELLS Angels program.

What if I don't Have Insurance but Need the Care Offered Through WELLS Angels?

  • If you do not have the ability to pay for services provided with this program, a limited amount of funding is available for individuals who qualify. You can find out more by contacting Megan Wisnieski, program coordinator (see contact information at bottom of page.
