Accessing Health Records on iPhone

Health Records on the iPhone is Now Available to Chase County Community Hospital Patients, the application offers a simple and secure way to access your medical data.

Chase County Community Hospital is proud to announce that patients can now access their medical records through Health Records on the iPhone which brings together hospitals and clinics with the Health app on iPhone to make it easy for patients to see their available medical data from multiple providers whenever they choose.

Chase County Hospital is excited to offer this opportunity to patients. To facilitate the flow of data to Health Records on iPhone, patients must first be members of the Chase County Community Hospital or Imperial or Wauneta Clinics Patient Portal. You can access this by going to our website at www.chasecountyhospital.com. Please click on the Patient Portal tab located on the home page of our website where you may choose self-enrollment or login if you already have your account set up.

Patients can access Health Records from within the Health app and can download their health records by selecting Chase County Community Hospital or Imperial or Wauneta Clinics and authenticating with their patient portal username and password.

Previously, patients’ medical records were held in multiple locations, requiring patients to log into each care provider’s website and piece together the information manually. Apple worked with the healthcare community to take a consumer-friendly approach and created Health Records based on FHIR (Fast Healthcare Interoperability Resources), a standard for transferring electronic medical records.

Now, patients will have medical information from participating institutions organized into one view, covering allergies, conditions, immunizations, lab results, medications, procedures, and vitals, and will receive notifications when their data is updated. Health Records data is encrypted and protected with the user’s iPhone passcode, Touch ID or Face ID.
